OFFICE VISITS (ADULTS, AGE 13 and OVER):

Initial Visit: $225

90 minutes: includes medical history, consultation, exam, acupuncture treatment and herbal consultation, if appropriate. PLEASE NOTE: we require a credit or debit card in order to book your initial visit, and we have a 48-hour cancellation policy for new patients.

Follow-Up Visit: $100

60 minutes, once per week or less frequently; includes consultation, exam, acupuncture treatment, herbal consultation, nutritional consultation, other modalities as needed per discretion of the acupuncturist.

Follow-Up Visit (when coming twice weekly on a regular plan of care, as the in pre-IVF protocol or for breech presentation): $80

Nutritional Consultation Only (Existing Patient): $80

Brief Re-Exam, Herbal Consultation Only/Sick Visit (15minutes): $55

Please Note that all patients under the age of 18 must be accompanied by a parent or guardian.

HOME VISITS:

Home Visit (90 minutes): $280  + Driving Fee

Includes medical history, consultation, exam, acupuncture treatment and herbal consultation, if appropriate.

We offer home visits to patients within a limited driving distance of our clinic; please call 818-392-8797 or email frontdesk@floatchinesemedicalarts.com if you have questions.  We can accept all credit cards at home visits, using Square.

 

SKYPE or PHONE CONSULTATIONS With Abigail Morgan, L.Ac., FABORM

Depending on your condition, and/or if you live outside of Southern California, you may be eligible for a remote consultation with our clinic owner and director, Abigail Morgan, L.Ac., FABORM.  (Our associate acupuncturists do not currently offer remote consultations.) The best and most thorough care is given in person, so the doctor can do a physical exam.  However, some conditions can be managed remotely, via Skype or Phone.  Abigail uses her skills of intuition along with her expertise as a practitioner of Traditional Chinese Medicine to give diagnostic impressions and come up with a plan of care.  She may also be able to refer to a Licensed Acupuncturist in your area if acupuncture is needed.

FORMS OF PAYMENT

We accept all major credit cards, checks, cash, and FSA and HSA (Federal and Health Savings Account) Cards.  Please note that bounced checks will incur a fee.  

INSURANCE

Payment in full is due at the time of service.  We are not a contracted provider with any insurance companies, and we no longer direct-bill for insurance, but if you have a PPO plan, we can give you a super-bill with the necessary codes, which you can send into your insurance company for reimbursement if your plan covers Acupuncture.  (HMO plans do not reimburse you for superbills.) If you would like to know if your health insurance plan covers acupuncture, call the Member Services phone number listed on the back of your insurance card and ask for a "Quotation of Benefits for acupuncture from a non-participating (a.k.a. out-of-network) provider." 

CANCELLATION POLICY

We require a minimum of 48 hours' notice to cancel a New Patient Appointment and 24 hours' notice to cancel a follow-up appointment. Call us at 818-392-8797 or send an email to frontdesk@floatchinesemedicalarts.com.  If you cancel with less than 24 hours of your scheduled appointment (see above), you will be billed in full.  We appreciate your understanding. 

ARRIVING ON TIME

At FLOAT, we do our best to run on time.  We are a small practice, providing one-on-one care.  We respect your time; please respect ours.  

Los Angeles traffic being what it is, PLEASE allow extra time to arrive at our office if your appointment falls during rush hour (i.e. 8:30-10am, 4-6pm).

New patients, please arrive at least 10 minutes before your schedule appointment time (20 minutes before if you have not yet filled out the initial paperwork).

Follow-up patients, please arrive at least 5 minutes before your scheduled appointment time, so that you can use the bathroom, have a cup of tea, or relax for a few minutes after driving.

We want you to get the full benefit of your appointment, and if you are late, you won’t have as long to relax with the needles or as much time to ask questions of your acupuncturist.

If you are more than 15 minutes late for your appointment time, we may not be able to treat you (there will not be time for a thorough exam and acupuncture treatment) and you will be charged in full for the appointment.  (Of course, emergencies happen, but please do your best to arrive on time.)
